The NP will work alongside the Attending physicians on the heart failure/post heart transplant consult service as they provide care for pre-heart transplant, post-heart transplant, ventricular assist device and advanced heart failure patients. The NP will be responsible for patient assessment and collaboration with the primary service as a key member of the multidisciplinary heart failure/heart transplant team.
The NP functions in an interdependent role by implementing the appropriate Standardized Procedures (SPs). By using the SPs the NP is able to change treatment regimen after observing signs and symptoms of illness, reactions to treatment, general behavior and physical condition and determines if they are abnormal findings (Ca Nursing Practice Act, subsection(b) of Section 2725). The NP will provide and coordinate the care of patients, provide patient/family education, promote safe continuity of care. The NP also participates in interdisciplinary and clinical rounds, provides education and support, as well as makes appropriate referrals and documents services rendered.
Standardized Procedure(s) outline the requirements for supervision. The Nurse Practitioner reports to the designated medical director for clinical practice and designated person for administrative issues.
Salary range: $84.82 - $121.13/hourly

- CA RN License
- CA NP License, State of CA NP Furnishing with Schedule II, National Certification (i.e. ANCC) in AG- ACNP (Acute Care)Specialty -required
- Master’s or Doctoral Degree in Nursing in Adult-Gero Acute Care NP Specialty -required
- NPI #, DEA #, Enrollment in PECOS, Enrollment in CURES -required
- Previous NP experience -required
- Cardiac and Critical Care experience -required
- Cardiac ICU experience - preferred
- BLS from AHA or ARC -required
- ACLS from AHA -required
- Ability to utilize excel, word and other software programs -required
- Ability to establish and maintain cooperative and collaborative working relationships with administrative, clinical and academic personnel. -required
- Skill in working independently and following through with minimum direction- required
- Ability to set priorities -required
- Demonstrate writing skills to compose logical, detailed and grammatically correct correspondenc -required
- Document patient care in a concise and comprehensive manner -required
