Primary Duties and Responsibilities

Using your advanced practice nursing skills, you will assess, coordinate care and provide education for a diverse patient population.  This will involve collaborating and consulting with a multi-disciplinary clinical/professional team as well as patients and families to ensure safe and effective coordination of care. You will:

  • Assess, coordinate, develop and implement individualized care plans
  • Utilize evidence-based tools for risk stratification
  • Perform utilization reviews, including authorization reviews
  • Ensure delivery of concurrent and post-hospital care
  • Initiate referrals, coordinate resource utilization, and evaluate post-acute care services
  • Participate in performance improvement projects/processes
  • Implement a comprehensive DC plan for high-risk patients

Salary Range: $63,800 - $127,000/Annually

Job Qualifications

We’re seeking a self-directed, assertive, creative problem solver with:

  • CA LVN licensure
  • At least three years of acute hospital experience required
  • Two or more years of utilization management, case management or discharge planning experience in an HMO environment required
  • Team leading or management experience
  • Systems planning and care management background in a high-volume setting
  • Proven resourcefulness with strong communication, diplomacy, organizational, prioritizing, and analytical skills
  • Ability to work effectively and collaboratively with interdisciplinary teams
  • Computer proficiency
  • Understanding of the functions of a large university teaching hospital
  • Ability to abstract and interpret medical information from patient records
  • Familiarity with the laws, rules, and regulations regarding utilization review and discharge planning of government programs such Medicare, Medi-Cal, and CCS
  • Ability to develop individualized case management plans that address physical, vocational, psychosocial, financial, and educational needs
