Primary Duties and Responsibilities

In this role you will provide administrative and patient-centered support to the Dementia Care Specialists (DCSs) and their patients/caregivers within the UCLA Alzheimer’s and Dementia Care Program. You will serves as a key liaison between patients, caregivers, DCSs, and referring physicians. Responsibilities include:

  • Assisting patients in navigating services within the health system
  • Facilitating timely communication among care team members 
  • Coordinating referrals to appropriate community and clinical resources. 
  • Follow-up through telephone outreach and accurate documentation in EPIC/CareConnect. 
  • Being professional, compassionate, and have strong organizational skills to support the delivery of high-quality dementia care.
Salary Range: $30.36 - $ 43.49/hourly 

Job Qualifications


  • Demonstrates proficiency with computer skills (MS Office Suite), document scanner and other software

  • Ability to learn electronic medical record system to document patient notes and produce reports

  • Ability to communicate clearly, patiently, professionally and compassionately via e-mail and print.

  • Oral and written communication skills to obtain and to convey information clearly, concisely and accurately.

  • Fluent in communicating verbally in English

  • Able to apply care management protocols to patients/caregivers, as appropriate.

  • Ability to analyze and relay potential concerns to Dementia Care Specialist, as appropriate

  • Demonstrates organizational and critical thinking skills

  • Ability to maintain confidentiality and discretion regarding sensitive materials and issues

  • Ability to meet deadlines and to follow through on multiple tasks with careful attention to detail and with minimum supervision.

  • Demonstrated writing skills to draft routine business correspondence and meeting minutes and to edit into final form.

  • Demonstrated editing skills to identify errors in grammar, syntax, punctuation and spelling.

  • Interpersonal skills to effectively develop and maintain cooperative working relationships with faculty, administrators, students and with elderly patients.

  • Ability to understand and to adhere to University and UCLA Health policies and procedures, especially as it pertains to patient and family confidentiality.

  • Attention to detail, in order to accurately enter and manage database for ADCP

  • Ability to identify and to prioritize based on needs assessment from information provided.
