Primary Duties and Responsibilities

Collaborating with patients, the Faculty Practice group, community resources, providers, managed care operations, the UCLA Health System and health plan, you will implement, coordinate, and monitor options and services to meet at-risk patients' healthcare needs. This will involve coordinating with providers, Comprehensive Care Coordinators, and staff to promote quality, cost-effective outcomes and appropriate level and mix of care. 

*This position is 25% flex-hybrid and includes significant field work (patient homes, community and office visits with patients)

In this role, you will also be responsible for:

  • Providing direct social work services, including assessment, intervention, and making recommendations to patients and their families
  • Participating in interdisciplinary activities
  • Ensuring compliance with departmental standards for documentation, continuous quality improvement, statistics, Code Of Ethics, etc. 
  • Keeping current on relevant social work healthcare knowledge 

Salary Range:  

MSW will be hired as a Clin Social Worker 2, LCSW will be hired as a Clin Social Worker 3

Clin Social Worker 2 Salary range: $99,785.52 – $119,203.92 Annual

Clin Social Worker 3 Salary range: $108,450.72 – $129,623.04 Annual

Job Qualifications

We're seeking a self-motivated, flexible, Clinical Social Worker with:

  • MSW or LCSW (MSW will be hired as a Clin Social Worker 2, LCSW will be hired as a Clin Social Worker 3)
  • One or more years of social worker experience in a healthcare setting
  • Demonstrated psychosocial assessment skills with patients/families
  • Background with clinical interventions and treatment planning
  • Excellent communication, relationship building, analytical and problem solving skills
  • Proven success working with interdisciplinary teams
  • Willingness to participate in at least one area of specialized practice annually
  • Strong organizational, multitasking and time management abilities
