Primary Duties and Responsibilities

This role will support the UCLA Alzheimer’s and Dementia Care Program by working closely with Dementia Care Specialists (DCS) to coordinate care for patients and their caregivers. Major duties include but are not limited to:

  • Conducts follow-up calls
  • Schedule appointments
  • Document patient interactions under the supervision of the DCS. 
  • Monitoring lower-acuity patients 
  • Performing needs assessments, and implementing appropriate program interventions. 
  • Gathers, analyzes, and manages large sets of clinical and survey data, while maintaining compassionate communication through phone and electronic records.  
  • Referring patients to community-based services
  • Supporting non-pharmacologic behavioral strategies 
  • Facilitating access to care across the health system. 
  • Collaborates with DCS and LCSW staff to ensure accurate patient acuity tracking and provides weekly updates on patient interventions.
Salary Range: $30.36 - $ 43.49/hourly 

Job Qualifications


  • Demonstrates proficiency with computer skills (MS Office Suite), document scanner and other software

  • Ability to learn electronic medical record system to document patient notes and produce reports

  • Ability to communicate clearly, patiently, professionally and compassionately via e-mail and print.

  • Oral and written communication skills to obtain and to convey information clearly, concisely and accurately.

  • Fluent in communicating verbally in English

  • Able to apply care management protocols to patients/caregivers, as appropriate.

  • Ability to analyze and relay potential concerns to Dementia Care Specialist, as appropriate

  • Demonstrates organizational and critical thinking skills

  • Ability to maintain confidentiality and discretion regarding sensitive materials and issues

  • Ability to meet deadlines and to follow through on multiple tasks with careful attention to detail and with minimum supervision.

  • Demonstrated writing skills to draft routine business correspondence and meeting minutes and to edit into final form.

  • Demonstrated editing skills to identify errors in grammar, syntax, punctuation and spelling.

  • Interpersonal skills to effectively develop and maintain cooperative working relationships with faculty, administrators, students and with elderly patients.

  • Ability to understand and to adhere to University and UCLA Health policies and procedures, especially as it pertains to patient and family confidentiality.

  • Attention to detail, in order to accurately enter and manage database for ADCP

  • Ability to identify and to prioritize based on needs assessment from information provided.
