Primary Duties and Responsibilities

This position supports the UCLA Alzheimer’s and Dementia Care Program (ADCP) by working closely with Dementia Care Managers (DCMs) to coordinate patient care, data management, and follow‑up activities. The incumbent manages and maintains program data, documents care activities in the electronic medical record, and communicates professionally and compassionately with patients and caregivers by phone and email.


The role includes scheduling appointments, conducting needs assessments for patients with lower acuity levels, identifying families in crisis, and connecting patients and caregivers to appropriate community resources. The incumbent reviews and processes patient documentation, supports data analysis and reporting, and collaborates with DCMs to ensure accurate patient acuity assignments. Regular meetings with the supervising DCM support coordinated care planning and continuous program improvement.


Salary: $30.36 - $43.49 hourly 

Job Qualifications

Required: 

  • Demonstrates proficiency with computer skills (MS Office Suite), document scanner and other software
  • Oral and written communication skills to obtain and to convey information clearly, concisely and accurately.
  • Able to apply care management protocols to patients/caregivers, as appropriate.
  • Ability to analyze and relay potential concerns to Dementia Care Manager, as appropriate
  • Interpersonal skills to effectively develop and maintain cooperative working relationships with faculty, administrators, students and with elderly patients.
  • Ability to understand and to adhere to University and UCLA Health policies and procedures, especially as it pertains to patient and family confidentiality.
  • Attention to detail, in order to accurately enter and manage database for ADCP
  • Ability to identify and to prioritize based on needs assessment from information provided.
Preferred:
  • Bachelors degree
