Provide enhanced care management by assisting unhoused clients in navigating systems of care and accessing resources that address social determinants of health.

Perform outreach and collaborate with a multidisciplinary street medicine care team to recruit, enroll, and provide case management services to qualifying individuals for medical and social services.

Collaborate with clinicians and community health workers to assess patient needs and provide tailored enhanced case management addressing both clinical health needs and social determinants of health.

Develop care plans and assist clients with linkages to medical, psychiatric, social, educational, housing, food, and other necessary services.

Travel to community sites as needed, working alongside program clinical teams, patients, caregivers, community partners, office staff, and other key stakeholders.
