Primary Duties and Responsibilities

The Department of Medicine is seeking a compassionate and patient-centered Patient Navigator to support individuals living with sickle cell disease throughout their healthcare journey. In this impactful role, you will serve as a trusted resource for patients by coordinating care, responding to phone calls and patient inquiries, facilitating timely follow-up appointments, and helping patients navigate the healthcare system. You will work closely with patients to address insurance-related barriers and connect them with available resources, while collaborating with physicians, clinic staff, and multidisciplinary care teams to ensure seamless, high-quality care. This position offers the opportunity to make a meaningful difference by improving access to care and enhancing the overall patient experience for individuals with sickle cell disease.

Hourly range: $26.42-$37.49

Job Qualifications

Required:
  • Ability to operate office equipment such as printer, scanner, copier, facsimile machine.
  • Previous office experience.
  • Experience working independently with excellent interpersonal and problem-solving skills.
  • Excellent writing skills and editing correspondence and written materials.
  • Ability to multi-task under deadline, balance competing priorities and solve problems creatively
  • Knowledge of PCs, windows operation system, and related word processing and spreadsheet software (such as MS Word, Access, etc.).
  • Ability to maintain confidentiality of documents.
Preferred:
  • Experience working with underserved populations, such as Sickle Cell patients
