Primary Duties and Responsibilities

The Population Health Pharmacy Program supports Population Health initiatives to increase the quality of care, while maintaining cost-effective outcomes through disease and medication management programs. The Pharmacy Technician is an integral member of the Faculty Practice Group’s Office of Population Health and Accountable Care (OPHAC) team and will work closely with the Pharmacist to execute strategies for the Population Health Pharmacy Programs as it relates to value-based care incentive programs. The Pharmacist and Technician team will seek to find opportunities for cost avoidance, improve quality of care, medication adherence, and work together to implement changes.

The Population Health Pharmacy Technician will assist the Pharmacist in monitoring and coaching patients on medication adherence and disease management to ensure health goals are met. A primary goal of this role is to enhance performance on population health measures related to medication management. Performance will be tracked by pharmaceutical STAR quality ratings. The Pharmacy Technician is also responsible for gathering information from medical records, excel spreadsheets, tableau, and other technology-based systems to support quality measure performance.

Salary Range: $26.55-$32.36/hourly

Job Qualifications

Required: 
  • Current California Board of Pharmacy Technician licensure in good standing
  • Knowledge of generic and trade drug names and pharmacy terminology
  • Minimum 2 year experience as a pharmacy technician in a managed care or retail setting
  • Ability to utilize Word, Excel and other software program
  • Capable of communicating prescription authorizations to pharmacists/medical personnel/pharmacies
  • Able to use pharmacy information systems and look-up patient data on hospital information systems
  • Ability to establish and maintain cooperative and collaborative working relationships with administrative, clinical and academic personnel
  • Skill in working independently and following through with minimum direction
  • Ability to set priorities which accurately reflect the relative importance of job responsibilities.
  • Demonstrate writing skills to compose logical, detailed  and grammatically correct correspondence
  • Document patient care in an concise and comprehensive  manner
  • Knowledge of efficient interviewing practices and techniques used in order to gather information.
  • Strong interpersonal skills with the ability to build relationships with providers and patients.
  • Excellent verbal and listening skills both in-person and on the phone required.
  • Ability to maintain quality, safety and/or infection control standards.
  • Excellent customer service skills required. Ability to be culturally sensitive and work with a diverse patient population.
  • Knowledge of medical records and appropriate documentation requirements.
  • Ability to maintain patient confidentiality required. Knowledge of HIPAA requirements.
  • Ability to remain calm and be resourceful in emergency situations required.
  • Ability to maintain a professional demeanor at all times.
  • Skill in listening perceptively, conveying awareness and responding to patients and visitors with health concerns using tact and discretion.
Preferred:
  • Prior experience in medication adherence programs
  • Experience with Epic EHR functionality
  • Capable of communicating prescription authorizations to pharmacists/medical personnel/pharmacies
