Primary Duties and Responsibilities

Come join our dedicated and caring team of Substance Use, Motivation, Medication Integrated Treatment (SUMMIT) professionals as a SUMMIT Case Manager. Use your expertise to make a difference in the community while growing your career. With guidance from our SUMMIT Director of Behavioral Health Services, the Summit Case Manager will work independently to coordinate the delivery of substance abuse treatment services to our patients in a primary health care clinic.  

The case manager will:

  • Support Summit patients after substance abuse treatment visits, arrange for follow-up appointments, and coordinate MAT appointments. 
  • Provide high-quality case management for patients experiencing challenges around homelessness, substance use, and attaining other resources, by linking them and helping them achieve their identified goals. 
  • Ensure that patients are linked to care by following up after missed appointments. 
  • Act as a liaison between the medical provider and the patient. 
  • Operate in a multi-disciplinary team and have patients referred to them from medical and mental health providers as well as community agencies. 
  • Work independently but consult on challenging cases with their supervisor.

Salary range: $29.64 - $39.13 Hourly

Job Qualifications

Required:
  • Bachelor’s degree in social work from program approved by Council on Social Work Education or equivalent combination of education and experience
  • At least 1 year of relevant experience
  • Knowledge of HIV/AIDS, community resources, health education methods, advocacy, harm reduction and general clinical knowledge.
  • Ability to create and maintain boundaries with clients.
  • Basic computer skills and the ability to complete required program documentation.
  • Excellent communication skills, public speaking skills, and leadership skills.
  • Ability to manage multiple deadlines and demands, and follow through on assignments.
  • Ability to work independently and ability to work effectively with people of diverse cultural and educational backgrounds.
  • Ability to discuss sensitive matters in a non-judgmental manner and to respect confidentiality. 
  • Working knowledge of diverse cultures and low income populations including the homeless.
